Two Steps for obtaining a personal CQ assessment feedback report.

Step 1:

	i.	Complete the profile information to activate an account
	ii.	Obtain email confirmation that your account is activated and you can move on to Step 2

Step 2:

	i.	Complete the assessment questions
	ii.	After answering the questions, you will receive your personal on-line CQ assessment feedback report, which you can download and print.
